New client intake

You do not need a referral to access services at ROOM TO BE. Referrals are welcome but not required.

To get started, simply complete the relevant intake form below and Fiona will be in touch with you shortly.

​

If you plan to use NDIS funding, once your intake form has been received, Fiona will liaise with you and/or your support coordinator to prepare a service agreement.

​

If you need assistance completing the intake form, you can contact Fiona directly (details in the footer) or book a complimentary 15-minute intake call.

​

You are also welcome to book your first appointment online, as the booking process includes completion of an intake form.​​

Adult intake form

To be completed by the client, or by a support coordinator (NDIS participants) if the client has given permission.

Adolescent Intake Form

To be completed by the client (if 14 or over) or the client’s parent/guardian.

Couples Intake Form

To be completed by each person in the couple.
